The 18-year-old who was found on fire on Sunday in south west London, has been named as Rachel Osei, the mother of a three-year-old boy.
Rachel ran into the street screaming, before collapsing at a level crossing near a Tesco, where she died with severe burns all over her body. Her son was staying with relatives at the time of the incident.
Police believe Rachel may have caught fire inside her house and are treating her death as unexplained. However their main line of inquiry is that her injuries may have been self-inflicted.
Her father Jonathan Osei was shocked by the incident and told reporters that he failed to protect her:
Rachel was at university training to be a sports teacher. She wanted to do well and provide for her son. When I heard about the fire I tried to call her on her mobile phone but there was no answer. I tried to protect her but I could not.
The young mother was living in a property owned by the Wandle Housing Association, which often accommodates single mothers.
A police spokesman confirmed that officers had been called to the address twice on the Saturday before she died and had spoken to Rachel. As a result of the contact the matter has been referred to the Director of Professional Standards and the Independent Police Complaints Commission.
